Send a check or money order to the clinic address listed on your statement. Please include your account number on the memo line for accurate processing.

When is my payment due?
Payment is due upon receipt of your statement. If you have insurance, your statement will reflect any remaining patient responsibility after your plan has processed the claim. If you need additional time, contact us before the due date to arrange a payment plan.
